Isn't there an actual address to send the forms to?

Im looking for the address to send in my form 5's to, there is the "submit to" section on the front right corner in DC to the head of the dept, but there is no actual address. Do I just label the envelope as it is stated on the form and pray the postal service is on the ball? There is no P.O. box or actual business number. One more question, do you guys normally ship standard mail or ship certified and not request a signature or request signature. Sorry for all the newb questions, but I do appreciate the input.

Yes, that address works. I put it all in a Priority Mail Envelope. No signnature, no confirmation, never had a problem.
