Why does my K9 have a U stamped on the left side of the Trigger Guard?
I thought on Smith&Wessons this meant a factory second or major repair?

I know it's used on the factory test-fires but I am not sure if the same symbol is also used on the refurbished ones.

The U stands for Factory Refurbished or Factory "Test Fires". My dealer sells a number of "Test Fires". I own one myself, a P40 Covert. It has been back to Kahr twice, for failure to go into battery, but now it is 100% reliable with 800 trouble free rounds. Kahr never questioned the need for repair.
My dealer sells "Test Fires" and "Refurbished" for about $100 less the the same model NIB. He also clearly identifies them as "Test Fires" or "Refurbished".
My understanding is that "Test Fires" may be Firearms that were sent out for "evaluation" ie: a gun writer or samples at the show like The Shot Show or the NRA Convention.
Refurbished are just as it sounds.
