C&R: Provide Rental Agreement??

I need to amend my C&R FFL as I just moved. I'm currently renting an apartment and question #20 on the "Application to Amend an FFL" asks for the owners name and a copy of the lease agreement.

Is this required when filing an amendment for a C&R?

Are you sure that application you are looking at is the proper way to change the address on an existing 03 FFL? I don't think its used for that type of "amendment". I think I recall the packet gave some instructions as to how to change an address and I don't recall it mentioning that Application as part of the process to do that.

I'm 100% sure it's the right form. I used it last time I moved. The last time I moved, I bought a house, so question #20 didn't apply.

I have a feeling it doesn't apply to those with a C&R license. But I thought I'd ask.

Does anyone have the an ATF/NLC email address that I can contact them with?
