Title says it all, I'm looking into getting a suppressor for my m4 and I know all about the tax and so forth but I can't find the forms. I looked here
http://www.atf.gov/dcof/index.htm
And while I'm sure it's staring me in the face I couldn't find it, I just need to find them so I can get the ball rolling

You will need to know the suppressor' relative information first to properly fill out the 5320.4.. maunufacture, cailber, length, model, serial #.
You need to pay for the suppressor, and if the SOT is worth a hoot, he will process & print the 5320.4 from aan on-line .pdf source for you and supply you with the (2) FD-258 fingerprint cards, and a compliance certificate, then you just take the 5320.4 to CLEO for signature, have them do prints while there.

Buy the can
fill out the form
and head to my local police station
Now do I pay the 200$ when everything is complete?
And thanks guys, I'm sure you get these dumb questions all the time but the darn search function is down and google was no help

If you're buy from a SOT, the process is as outlined by Capt.
If you're buying FTF, you'll have to meet with the owner, get the info, fill out the form 4, get printed on FD 258 in duplicate, attach passport photos in duplicate, get a signature from the CLEO, DA, judge or other approved individual, include a check or MO for $200 payable to ATFE and mail it in.
If everything is in order, you get an apporval back in 30-120 days and can take delivery.
You pay the $200 tax when you send in the form 4, fingerprint cards and passport photos.
Your dealer will walk you through the whole process.
Fill out the form 4.
2 sets of fingerprints,Picture of yourself a.CLEO sign off.
Send all in with 200$ and usually within 30 days your good.
If you want to do it without picture fingerprints and without the CLEO.Or maybe you live somewhere the CLEO refuses to sign for anyone you have hope.
Regardless this is the way to go.
Use quicken will maker, unless you wanna pay someone and set up a revocable trust.Must be revocable so that you can add toys to it as you get them. You will not need fingerprints, picture , CLEO sign off .Still gotta pay the 200$:crying:
Took me 10 minutes to set up my trust.the program walks you through every step .you really just answer some questions. You will need to check to see if you must register trusts there. In my home state you do not need to register.
you send in the trust ,can info and the 200$ .
My trust is only 3 pages.
Almost forgot ,you must get the trust notarized.
Revocable trusts are the way to go for all your class2,3 toyz...
I learned all I needed to know at www.silencertalk.com I was recently approved on my first suppressor purchase going the trust route

This may help some:

Just understand that you can do trust or corporation ownership as well. If you do either one of those then you don't need the signature card, passport photos or the CLEO's signature.
The Maj is correct and a great guy to deal with. He talked me through my first NFA purchase, A Walther P22 with TAC 65. I purchased combo from him, mailed my forms to BATFE 10/23 went pending 11/21 and approved 11/29. Did this on a RLT with no CLEO sign off no finger prints and no passport photo. This has got to be the best 429 bucks + 200 tax stamp + shipping and transfer fees I have ever spent................Well there was this one time in Vegas :)
