I asked over at ARF, but haven't gotten a reply in a day, so I'll see if anyone here is willing to help. TIA.
Okay, I've got my trust made up using Willmaker, now I just have to fill out my Form 4. I did one already but it wasn't with a trust. I just need to make sure I get this right, and I'm sure you guys know what's going on here and most are readily willing to help.

Box 2a: Transferee's name and address --
Do I put the name of the trust -- Sterling blah blah Revocable Living Trust along with my present address?
Boxes 13: the section where you answer all the "yes/no" questions -- fill them out as normal?
Box 15: Transferee's Certification --
Do I sign my name as I would normally sign my name on any legal document?
Box 16: I know there's no photograph needed, so I'm good there.
Box 17: Law Enforcement Certification -- Should I just put "N/A" in all the blanks since this is not needed?
Lastly, on the 2nd form, the "Certification of Complaince with 18 U.S.C. 922(g)(5)(B) --
It has in box 1: Name -- Last, First, Middle Initial -- OR Name of Corporation, Partnership, or Association (if applicable).
Is this to be filled out with my name as last/first/middle or should the name of the trust be put where it asks for the corp/paternship/assoc.?
