ammo malfunction

i have a couple of pics of a bullet that blew up in my buddies 1911 we cant figure out what happened . the gun didn't malfunction before this happened when the round went off all the burning powder came out the rear of the slide right in his face (good thing he was wearing his glasses) the gun is ok and went right back to running with no problems. so has anyone seen this before ?

What brand and weight? I'm going to guess Blazer Brass 230 Gr FMJ.

It appears the case neck did not have enough tension to keep the bullet from setting back.

Huge setback!
Also note the ding in the bullet, looks like it is slamming into the edge of the feedramp. Might need a good 1911 smith to give it a look over. Don't let someone just start butchering on the feedramp.
