I like Milt Sparks and Seth's Custom Leathers for concealed carry. Do other custom leathersmith products or good off-the-shelf holsters work? Of course they do, I also have Bianchi, Galco and Safariland. However, I like the people at Milt Sparks and Seth's and that's why I favor their products over Mitch Rosen/Del Fatti/Alessi/Kramer.
Nowadays I like to develop relationships with people and I don't know Rosen, Del Fatti or Alessi. I spoke with Kramer before at a gun show and his holier-than-thou attitude turned me off. His stuff is good but not better than Milt Sparks or Seth's. So, for now for concealment holster, I'll stick with Milt Sparks and Seth's. Though Seth's is making something for my M1911s that aren't for concealment.
Anyhewww, I carry my M1911 concealed in a Milt Sparks 55BN (the old Bruce Nelson Professional with slight mod), the IBAK (Improved Baker) or sometimes IWB style with the Executive Companion with the NLG (Nelson Legacy) coming. I'm not an IWB fan though I think that my Colt Detective Special might be more comfortable for me IWB. If that's the case then Seth's will have to do something for me.
I like the 55BN so much that I had holsters made for my P7PSP, P5 and 92FS. Sometimes it's hard to improve on a classic that was well designed from the start.
I also like Seth's Paladin a lot too. Super comfy and hugs the body tight. It makes a wide gun like the SIG P228 disappear around my fat belly.

Whatever works the best for you. I've kind of gotten attached to a D.M. Bullard Bodyguard.

I agree on the Axiom. It has a smooth draw, conceals well, comfortable to sit in a car with, and easy to snap on and off.
I find I wear it more than my VMII now when concealing because its just as easy to conceal as long as the shirt isn't too short.
The only potential drawback I can see is retention since it doesn't have the reinforced mouth.
My only regret on the Axiom is that I waited so many years to try one.

I was going to go with a Milt Sparks leather holster for daily carry to compliment my Blackhawk Serpa that i use in competitions but i've been looking at Raven Concealement alot... it looks like that may offer my next OWB solution.

Fobus holsters are junk and you're better off throwing your money out the window. Nobody seriously carries these things.
This individual shows how fragile they are...
To the OP, get the best holster you can afford like FN said, what works for you will be different from everyone else. Personally, I own or have tried Milt Sparks, Alessi, and Tucker that have worked for me. MS VM-2 is my preference.

Milt Sparks Summer Special 2 is a great IWB holster, & for a shoulder rig the Bodyguard rig from Alessi Leather is another one hard to beat 1911 holster, I own both of them & have no problems with either one.

I like Wilson Combat Pro. It's available and holds the 1911/Commander just high enough to keep the pistol conceal under a short outer garment, but not to high to draw.

I think I have 4 or 5 holsters for my 1911 now. My favorite for IWB is my Milt Sparks Versa Max II
Before that for IWB I used a Milt Sparks Watch Six Dual Talon. I don't think they even make that version any more. I also have an old Dillon I use for OWB that I like but I can only use that when I am wearing a long coat or something.

I have an Axiom coming for a Commander size. I like the IBAK, but I've begun to appreciate quick detachable holsters.
I'll have to compare the Axion to the Paladin and see. Interesting that the Axiom doesn't have a reinforced mouth. I'll have to ask the Milt Sparks people about that.

They like me, so my orders had been more around four months. That's still going to be end of the year. Oh well, it's not like I'm short of holsters.

I like my Milt Sparks holsters.
Axiom. Best OWB I've ever owned. (and I've owned a few.)
VM II Commander size
VM II Government size with Kydex clips making the holster tuckable.
I also like my MTAC.
